INCIDENT REPORTING

Generally there are two types of incidents to be reported.

 

Work Health and Safety Incidents

 

These are incidents that arise out of the conduct of a business or undertaking. WHS incidents for the most part are incidents where a worker is at risk or is injured carrying out their duties or travelling between worksites. 

 

When an injury occurs to a worker, this leads to a workplace injury management process involving doctors, allied health professionals, rehabilitation providers and the workers compensation insurer.

 

The other type of WHS incident is when people other than workers are put at risk or injured by a worker or by work carried out by our workers.  Such as a worker creating a trip hazard.

General Incidents

​

These are incidents that involve clients, participants, customers, students but not involving our workers or our work.  This could include a client being found by their worker to be ill and in need of urgent care or a student acting out and upsetting classmates.

​

There are differing requirements for who specific types of incidents are reported to depending on the person involved.

When such incidents occur workers are required to:

​

  1. Call emergency services if required first

  2. Call the office, supervisor or manager  - immediately - do not leave a message, speak to someone

  3. If a Vworker app user - log in  Visual Care as an initial report 

  4. Complete a General Incident Report form PART 1, save and send to your   supervisor/manager

Complaints

​

If an incident also involves a complaint a formal complaint form will also need to be completed
